Players can defeat their rival (their opponent in a two-player game, or a randomly determined player if playing with three or more players) by reducing their prestige to zero, defeating all their active vampires, or by gaining thirteen agenda points. If a player is eliminated by someone other than their rival, the winner is the player with the most agenda points at that time.

Each player starts the game with twenty Reputation, which serves as their life total. Reputation is also used to recruit vampires, utilize abilities and effects, and can be gained or lost through various means during the game, creating an interesting balance between using prestige for an advantage and maintaining enough to avoid defeat.

The core set includes four clans: Brujah, Malkavian, Toreador, and Ventrue. The Wolf & The Rat Expansion adds two new clans: Gangrel and Nosferatu. The Blood & Alchemy Expansion introduces two additional clans: Tremere and Thin-bloods. These new clans bring with them gurus of blood conjuring and mixers of chemical powers, accompanied by unique abilities.
